All you need is:

A copy of your E&O declaration page
2) Completed
W-9
3) Two signed copies of
AMW's Producer Brokerage Agreement
Send all completed documents to our corporate headquarters:

Remember, you will need to sign
TWO copies of the Producer Brokerage
Agreement and send BOTH COPIES to our office, along with the copy of
your E&O declaration page and completed W-9. We will send you
a fully signed producer agreement when all documents have been
received.
