It's as easy as one, two, three!

All you need is:

1)  A current copy of your E&O declaration page

2)  Completed W-9

3)  Two signed copies of AMW's Producer Brokerage Agreement

 

An approved representative of your company will need to sign TWO copies of the Producer Brokerage Agreement.  Send BOTH COPIES to our office, along with a current copy of your E&O declaration page and completed W-9.  We will send you a fully signed producer agreement when all documents have been received.
